If you have a complaint about the pay-off, customer service at the racetrack or the facility, and any other complaint regarding the racetrack or the facility, the teller has complaint forms to fill out. Do not throw away any tickets that are related to the dispute.
If you have a complaint about the teller, first contact the track that employs that individual. If you are dissatisfied with a racetrack response to your complaint, you may submit your complaints to the Department. The complaint form is located on the Forms page of the website. You may e-mail your complaint to the Racing Division at PublicAffairs@azgaming.gov or print out the form to fill out and mail to: Arizona Department of Gaming, Racing Division, 1110 W. Washington, Suite 450, Phoenix, AZ 85007, or you may fax your complaint to the Department at (602) 255-3883.
